Medical Questions: If you are an established patient of Dr. York, you may e-mail medical questions to jyork@texasneurologycenter.com. Please remember, however, that this form of communication is not appropriate for use in an emergency. Depending on vacation/holiday schedules, your e-mail could go unanswered for up to two weeks.
Appointments: To schedule an appointment, please e-mail teresa@texasneurologycenter.com. Include your name, your medical issue, the name of your referring doctor, your insurance provider, your phone number with area code, and in general the time of day or day of the week you would prefer. Someone will contact you.
Medication Refills: For prescription refill requests, please ask your pharmacy to fax a request to us at 744-1654. If you are using a new pharmacy, e-mail us at teresa@texasneurologycenter.com. Please include your name, your medication name, strength, and how many times per day you take the medication. Also include the name of your pharmacy along with a fax number or phone number.
Billing Questions: E-mail your specific question, along with your name, birth date, telephone number with area code, and insurance provider to sheri@texasneurologycenter.com.
Medical Records Requests: Please e-mail diane@texasneurologycenter.com your name, date of birth, and where you would like the records sent. There is a $25 charge for records sent directly to a patient. There is no charge to send your records to another doctor’s office.
